Preparing for Your Appointment
Please arrive 15 minutes early for registration and initial measurements.
Bring a valid OHIP card to every visit. If your card is invalid, the appointment will be rescheduled
Newborn OHIP numbers from the hospital are temporary; register promptly for a permanent card
To renew a OHIP card click the following link
Please bring a list of of current medications and supplements (including dosages)
Appointment Timing & Late Arrivals
Appointments for new patients are typically 60 to 90 minutes to allow for thorough assessments
Follow up appointments are typically 30 minutes
If you arrive more than 15 minutes late for your visit, the appointment may be rescheduled
Virtual Video Appointments
Sprout Kids Clinic is happy to be providing virtual video appointments routinely, however video appointments never replace in person medical care and are always used in combination with in-clinic visits
Video appointments use the Medeo system (www.medeohealth.com)
Reminders & No-Show Policy
As a courtesy, Sprout Kids Clinic sends appointment reminders via email, please confirm or cancel to reschedule
Sprout Kids Clinic requests a minimum of 48 hours for appointment cancellations
A no-show fee of $50 may be applied for cancellations within 48 hours of appointment time or missed appointments
Uninsured Services & Payments
Some services are not covered by OHIP. These are called “uninsured services” and will be billed directly to the patient. These services range from $25-$100 per service.
Disability tax credit forms will only be submitted via online submission
Forms (for school, service animal, Make-a-Wish etc.)

Sprout Kids Clinic primarily communicates through secured message via Medeo

Zero Tolerance for Aggression/Abuse
Our staff and physicians deserve a safe environment.
Aggressive or abusive behaviour—including threats, verbal abuse, derogatory remarks, or swearing—will not be tolerated in person, by phone, email, or text.
